Lateral Extra-articular Tenodesis: The Onlay Technique

چکیده
Tibial anterolateral rotary instability associated with anterior cruciate ligament (ACL) tears is a well-documented and difficult problem long history of solutions. The lateral extra-articular tenodesis (LET) has undergone multiple refinements in technique, largely focusing on the femoral site insertion using either an interference screw versus staple for adequate fixation. In this article, we present improved technique suture anchor as alternative to or secure fixation insert LET graft onto femur. This diminishes chance ACL tunnel–LET drilling convergence, minimizes footprint required fixation, allows surgeon dial correct tension necessary augmentation reconstruction.

Extra-articular lateral reconstruction technique.
This article describes an anterolateral reconstruction procedure that, when used in combination with an intra-articular anterior cruciate ligament (ACL) reconstruction, restores rotary and anterior knee stability. We believe that failing to recognize lateral instabilities and to perform an extra-articular reconstruction is an under-recognized cause of failure of ACL reconstruction. We also desc...
متن کاملIntra-articular Anterior Cruciate Ligament Reconstruction With Extra-articular Lateral Tenodesis of the Iliotibial Band
An increasing concern has been given to the rotation stability of the knee in the setting of an anterior cruciate ligament (ACL) reconstruction. This growing interest stems from a better understanding of the rotational stability of the knee afforded by the identification of the anterolateral ligament. Previously, a residual abnormal pivot-shift test had been found after an anatomic single-band ...
متن کاملBiomechanical Results of Lateral Extra-articular Tenodesis Procedures of the Knee: A Systematic Review.
PURPOSE To systematically review and compare biomechanical results of lateral extra-articular tenodesis (LET) procedures. METHODS A systematic review was performed using the PubMed, Medline, Embase, and Cochrane databases. The search terms included the following: extraarticular, anterolateral, iliotibial, tenodesis, plasty, augmentation, procedure, reconstruction, technique, biomechanics, kin...
متن کاملExtra-Articular Lateral Tenodesis for Anterior Cruciate Ligament Deficient Knee: A Case Report
We present the case of an extra-articular lateral tenodesis for an anterior cruciate ligament (ACL) deficient knee. A 46-year-old male patient sustained an ACL graft rupture after a motorcycle accident. He complained of rotational instability and giving-way episodes. His previous graft was fixed by an intra-articular femoral staple that was not possible to remove at the time of the ACL revision...
متن کاملExtra-articular Mimickers of Lateral Meniscal Tears
CONTEXT Lateral meniscus tears are a common entity seen in sports medicine. Although lateral-side knee pain is often the result of a meniscus injury, several extra-articular pathologies share signs and symptoms with a meniscus tear. It is critical for the clinician to be able to identify and understand extra-articular pathologies that can present similar to a lateral meniscus tear. EVIDENCE A...
